What to Bring on the
Day of Surgery

Persons under 18 must have a parent or legal guardian with them to
sign the consent forms.

Driver’s license & all medical insurance cards
Diabetic medications (insulin/supplies/oral meds)
Inhalers
Medication – if requested in your pre-surgical call
CPAP machine, if you have one
Any medical equipment – including oxygen tank or compressor with cord if you are on home oxygen
Paperwork/images received from your surgeon’s office
Family member or friend to wait for you and to transport you home
